Required Documents for Registration
Birth Certificate  - Child must be 4 years of age for Pre-K or 5 years of age for Kindergarten on or before September 1st
PK Only - Proof of income
Shot Records
Social Security Card
Parent's Photo ID
Last Report Card and/or Withdrawal papers

Proof of Residence
If you live in a House - Current Water, Electic or Gas (Utility bill)
If you live in a Apartment - Current First and Last page of lease with signatures and ALL children must be listed on lease along with parent(s) or Current utility bill
